The transcribed reports
are formatted as per the specifications mentioned in the templates.
The template bears a proper structure of format, content,
spellings and grammatical usage. Reports received are transcribed
accurately to reflect the meaning intended by the Dictator.
Reports can however be further interpreted and edited by the
Transcriptionist.
During the transcription process,
the patient's right to confidentiality is strictly maintained.
Reports are completed and submitted in adherence to time deadlines.
Standard turnaround time to submit transcribed medical records
is 12 hours or as specified by the client at a mutually agreeable
time with the Company.
All the medical documents received
for transcription purposes are checked for any blanks or questions.
These are flagged or marked with a red underline or with comments,
which are directly inserted in the received document. This
allows the Transcriptionist to extrapolate directly in the
file while comprehending about a question or a blank.
The Editor then deletes the
comments or red underlines after the problem has been solved.
If a blank still remains, then the client is provided with
a cover sheet, which details the specified problems.
In the process of transcription,
the Transcriptionist and the Editor, both utilize English
and Medical spell checkers and dictionaries for correcting
spelling errors in the documents. A thorough spell check is
run on each file prior to saving it. An Account Representative
then gets on to work to verify correct formatting of the reports.
When a new account of a report is opened, the entire transcription
work is edited by a well-trained Editor, the report account
is given to the Transcriptionist only after the Transcriptionist
is confident on taking up the transcription work.
The following items are
checked during proofreading of medical files received: Grammatical
errors, punctuation marks and medical terminology used in
the document. Guidelines established under the 'AAMT Style
Guide for Medical Transcription' are strictly followed in
the transcription procedures.
